Long Term Acute Care


      The Long Term Acute Care Hospital at Infirmary West is the only hospital of its kind in the region. It is a hospital specifically designed to meet the special needs of patients requiring extended acute medical care and rehabilitation. It provides specialized care for patients who suffer from neurological disorders, including stroke and Parkinson’s disease, respiratory conditions, cardiac- related disorders, trauma, spinal cord injuries, burns, cancer, and any illness requiring extensive medical care.
      Care at Infirmary Long Term Acute Care Hospital is patient centered, individualized, and provides flexibility to adapt to each patient’s changing needs. We utilize an interdisciplinary medical team approach in order to optimize the quality of care to each patient.

What is a Long Term Acute Care Hospital?

      A long term acute care hospital is truly an extended-stay facility for patients suffering from chronic or complex medical diseases and injuries that no longer meet the criteria for a regular hospital stay.

In addition, we provide education to both the patient and family members, and provide assistance in planning for continuing needs after discharge from the hospital.

      Our team includes physicians, nurses, physical therapists, occupational therapists, speech therapists, recreational therapists, respiratory therapists, social workers, dietitians, pharmacists, and radiologists.

The Role of the Patient and Their Family

      We consider the patient’s family members and personal physician vital members of our team. Their participation in the program is essential to the success of the care provided. Education for the patient, as well as the family is an important part of the healing process and we depend on family members to participate in training sessions to help ensure proper home care following the patient’s discharge.
      Families are provided with weekly updates on the patient’s progress and family conferences are scheduled, as needed, to discuss concerns and expectations.

Admission Criteria and Expectations

      Referrals to Infirmary Long Term Acute Care Hospital can be made by physicians, social workers, case managers, home health providers, caregivers, or family members. The assessment procedure is initiated by calling our referral assessment coordinator at 251-660-5619.
      An on-site assessment will be performed to determine if the patient can benefit from our medical and therapeutic services. This assessment will consist of a complete review of the patient’s medical chart, relevant family/social history, and the patient’s healthcare coverage.
